Frequently Asked Questions

How Do I Get Started?

To refer a child for screening and evaluation, refer to the below contact information for the county the child resides in or complete our online form.

What age can children be referred?

All children birth to five years of age can be referred for a screening or evaluation.

Who can make the referral?

Anyone can make a referral regarding an infant, toddler, or preschooler who may show signs of a developmental delay in how they: see, hear, talk, walk, respond to others, play, and learn.

How is a child evaluated?

Most screenings are completed at the home. During the screening an intake specialist will complete a questionnaire and answer your questions. If the results show delays an evaluation is offered.

When possible, evaluations are conducted in the family’s home community. Current evaluations sites are located in Bend, Redmond, Prineville, and Madras.

Evaluations are completed by specialists with the input and assistance of the those who are close to the child. This input plays an important role in collecting accurate information. Evaluations include an observation of the child in a comfortable setting, in-depth interviews with family members and a standardized assessment of a child’s developmental areas of concern.

What happens if a child is found eligible for Early Intervention services?

If a child is found eligible, the family and a team of professionals work together to identify the child’s current levels of development and what skills the child needs to learn. Team members identify how the child’s educational and family goals will be met. This is called an Individual Family Service Plan (IFSP). This plan identifies goals for your child and details services.
